摘要
目的 观察肾移植术后病人血小板聚集功能的变化。方法 采集枸椽酸钠抗凝静脉血 ,以自身PPP调节零点 ,在PRP中分别加入ADP、AA、Col和Adr诱导剂在搅拌磁棒的搅拌下 ,以NJY - 2四通道血液聚仪测试 5 0名健康志愿者及 5 0名肾移植病人的血小板最大聚集率 ,全部试验于离体后 4小时内测试完毕。结果 由ADP(2 .0 μmol L)、AA(1.84mmol L)、COL(2 .0 μg ml)和Adr(0 .5 4μmol L)为诱导剂诱导的血小板最大聚集率在肾移植术后病人组分别为5 9.5± 10 .2 %、82 .4± 12 .4%、73.4± 14.8%和 6 6 .9± 12 .1% ;其在正常对照组为 49.0± 13.4%、46 .9± 12 .7%、41.8±12 .8%和 5 1.4± 13.7%。肾移植术后病人由AA和Col诱导的血小板聚集功能与正常对照组比较有非常显著性差异(P <0 .0 1) ;由ADP和Adr诱导的血小板聚集功能与正常对照组比较有显著性差异 (P <0 .0 5 )。结论 肾移植病人血小板聚集功能增高导致血液循环及微循环障碍 ,增加术后血栓形成的危险 ,应当引起足够的注意和重视。
Objective To observe the changes of platelet aggregate function with the patients after renal transplantation. Methods Collect anti-coagulation vein blood and adjust zero by self poor platelet plasma,and then adding induce reangents ADP?AA?Col and Adr and mix well countinually,detected the maxium rate of platelet aggregation in 50 health volunteers and 50 patients after renal transplantation by NJY-2 four channels blood coagulate and aggregate meter,all these tests must be measured within 4 hours after blood collections. Results The maxium rate of platelet aggregation reduced by ADP(2.0μmol/L)?AA(1.84mmol/L)?COL(2.0μg/ml)and Adr(0.54μmol/L)in patients after ranal transplantation were 59.5±10.2%?82.4±12.4%?73.4±14.8% and 66.9±12.1%.And they were 49.0±13.4%?46.9±12.7%?41.8±12.8% and 51.4±13.7%in normal controls.The platelet aggregate function induced by AA and Col have quite markedlty difference between the patients after renal transplantation and normal controls(P<0.01),and there were markedly difference induced by ADP and Adr(P<0.05). Conclusion The increase of platelet aggregate function with renal transplantation may cause barrier of blood circulation and microcirculation and raise the risk of thrombosis form.Ought to give rise to sufficient attention and to be taken into account.
