摘要
目的:旨在通过超声心动图新技术,定量分析干扰素治疗DCM后其心功能的变化。方法:52例住院和门诊DCM病人,随机分为两组,干扰素治疗组31例,平均年龄41.0±17.1岁;常规治疗组21例,平均年龄38.5±19.2岁。测量舒张末期面积(EDA)、收缩末期面积(ESA)、面积变化分数(FAC)、舒张末期容积(EDV)、收缩末期容积(ESV)、射血分数(EF)及采集二尖瓣水平、乳头肌水平、心尖水平CK图象并进行积分。结论:DCM病人在常规治和干扰素治疗后,其左心室收缩末期和舒张末期内径缩小不明显。干扰素加常规治疗可以提高左心室EF值,与常规治疗比较有显著差异。干扰素加常规治疗可以提高左心室收缩功能,增加左心室收缩期位移距离,与常规治疗比较有显著差异。超声心动图技术可以定量左心室收缩功能的动态变化,具有简便、准确的特点,易于临床应用。
Objective: This study was aimed at quantifying the changes of cardiac function with interferon -α through 1 month, 3 month, and 6 months theropy by new echocardiographic technique. Methods: Fifty two patients with DCM were randomized into two groups: one group were treated with interferon - α, and the other group treated with conventional thera-py . The AQ images were collected in apical four - chamber section, the data of EDV, ESV, EF, EDA, ESA, FAC were recorded. The CK images were collected on mitral plan, papillery plan and apex plan. The echocardiographic examination were performed four times before and after treatment. Results: The size of left ventricle was not decreased obviously in two groups. All the data of size of LVDs and LVDs showed no significant difference between these two groups. The difference of LVEF between two groups in 1 month and 3 month after treatment was not significant(P >0.05), but there was a significant differ-ence between two groups in 6 month after treatment (P=0.03). The difference of FAC between two groups was not signifi-cant through the all follow-up stages. Conclusion: The size of left ventricle were not decreased obviously in control and inter-feron groups after treatment. The increase of LVEF in interferon group was more markedly than that in control group, and a significant difference was showed in 6 month after treatment. The contractive function of left ventricle accoding to CK tech-nique was increased gradually in two groups, also it was significant difference between two groups in 6 month after treatment. Echocardiographic techniques such as AQ and CK may be the very useful methods in quantifying cardiac function of left ventri-cle, and for clinical diagnosis.
