颈椎手术围手术期处理的价值探讨

目的:观察颈椎手术前后的非手术治疗对影像学所反映出的颈椎生理弧度改变的影响,阐明围手术期的非手术治疗是手术取得较好效果的基础。方法:除常规准备外,强调病人平卧硬板床,项部垫凸枕,使颈椎呈轻度过伸位持续1周后施术,术后维持该体位,随访并影像学复查。结果:术中、术后可不同程度恢复和保持颈椎生理前凸。结论:手术是椎管减压的最直接途径,但由于手术本身对颈椎稳定性容易造成破坏,给术后病人颈部功能的康复造成负面影响。本方法从颈椎及其周围环境方面,为恢复或保持颈椎生理曲度和稳定性以及术后颈部功能康复提供了依据。

Objective:To observe the influence of treatment without operation before and after operation on the physiological curve of cervical spinal, and to expound that the treatment without operation is a foundation for better curative effect. Methods: Besids the routine work the patients must be made lie stretched out on the plank bed and the neck must be rest on a convex pillow,so that the cervical spinal can be extended. This posture must be kept for a week. Results: The physiological curve of cervical spinal was recovered and Kept on various extent after operation. Conclusion: Operation is one of the most direct way to reduce pressure of vertebral canal, but it is liable to destroy the stability of cervical spinal and exert harmful influence to the function of the neck after operation . The above-mentioned methods can provide grounds for keeping the physiological curve and stability of cervical spinal and recovering function of the neck.
