平板运动试验、QT离散度与冠心病的相关性分析 被引量:6

摘要 目的 探讨平板运动试验和 QT离散度 (QTd)对反映冠状动脉病变的价值及应用前景。方法 与冠状动脉造影对照分析 72例临床诊断或疑诊为冠心病患者的平板运动试验结果及运动前后 QTd的变化。结果  1冠心病患者运动前的 QTd明显大于冠脉正常者 ,达到最大运动量时 QTd较运动前更明显增大。 2多支冠脉病变平板运动试验、QTd的阳性率高于单支病变 ,QTd的阳性率高于平板运动试验的阳性率。 3平板运动试验、QTd二项均阳性比其单项阳性特异性、阳性预测值高。结论 同时行平板运动试验。 Objective To investigate the value and the application prospect of using exercise treadmill test(ETT) and QT dispersion (QTd) to reflect coronary artery d ersease.Methods Compared with coronary angiography,the results of ETT and the changes of QTd before and after exercise were analyzed in 72 patients who were diagnosed or doubted coronary heart disease(CHD).Results ①QTd before ETT were much larger in CHD than in normal cornary,especially larger when exercise achieved maximum.②The positive rates of ETT and QTd in patients with multivessel lesions were higher than those in patients with sigle vessel disease,and the positive rates of QTd were higher than of ETT in CHD.③When ETT and QTd were both positive,the specificity and the positive predictive value of CHD was more significant.Conclusions Using both tests at the same time to diagnose CHD showed higher specificity than using either test.
