肝移植术后胆道合并症的多层螺旋CT诊断 被引量:3

摘要 目的 探讨多层螺旋CT在原位肝移植术后胆道合并症的诊断价值。方法 对 2 8例肝移植术后病人多层螺旋CT检查进行回顾性分析。结果  9例病人术后发生胆道合并症 ,占各种腹部合并症的首位。其中吻合口狭窄 2例 (7.1% ) ,胆管漏 6例 (2 1% ) ,胆管结石 3例 (11% ) ,肝内胆汁瘤 3例 (11% ) ,4例病人多种胆道合并症同时存在。结论 多层螺旋CT多期增强扫描能够全面的显示肝移植术后胆道合并症的各种表现 ,但对胆管漏 ,胆管狭窄程度的判定有一定的局限性 ,应配合“T”管造影。 Objective To assesse the value of multi slice helical CT in the diagnosis of biliary complications after orthotopic liver transplantation. Methods Multi slice helical CT(GE Lightspeed) of 28 patients undergoing liver transplantation were retrospectively reviewed. Results 9 patients developed biliary complications,which ,of all kinds of abdominal complications,rank first.Among oosunjects,2(7.1%) patients had biliary stenosis,6(21%) biliary duct leakage,3(11%) biliary calculus,3(11%) intrahepatic biloma,and several complications co existed in 4 patients. Conclusion Multiphase scanning with multi slice helical CT can display fully varied biliary complications after orthotopic liver transplantation..But there is limitation in detecting biliary duct leakage and determining the extent of biliary stenosis.so helical CT should combined with T cholangiography.
