球结膜环切及角膜溃疡浅层切除术治疗蚕蚀性角膜溃疡

Treatment of Peritectomy and Shallow Layer Resection of Corneal Ulcer with Mooren's Corneal Ulcer

目的:为观察球结膜环切及角膜溃疡区浅层切除治疗蚕蚀性角膜溃疡的疗效。方法:治疗组32例,在显微镜下,于局麻后,切除角膜周围约8～10mm宽的球结膜及角膜溃疡区浅层组织。术后用2％乙酰半胱氨酸及抗生素眼药水滴眼。并以强的松、环磷酰胺口服及2％乙酰半胱氨酸、抗生素眼药滴眼等常规方法治疗30例作对照。结果:治疗组32例,治愈28例,好转4例,治愈率87.50％。对治愈的28例进行1年的随访观察,复发2例,复发率为7.14％。对照组30例,治愈21例,好转7例,无效2例,治愈率70.00％。对治愈的21例进行1年的随访观察,复发7例,复发率为33.33％。结论:球结膜环切术治疗蚕蚀性角膜溃疡具有疗程短、疗效好、复发率低的优点。

Objective: To determine the effect of Mooren s corneal ulcer in treatment of peritecto my and shallow layer resection of corneal ulcer. Methods: In the operating microscope, 32 cases with Mooren' s corneal ulcer after hocusing of local were resected the 8 ~ 10mm wide bulbar conjunctiva organization around cornea and the shallow layer of corneal ulcer organization. After operating,2% acety lcysteine and antibiotic eye drops were dripped into the eye.30 cases with routine pharmic treatment were served as a control group.Result:In operation group,28 cases were cured,4 cases were mended and the rate of cure was 87.50% .The cured 28 cases were followed up for one year,2 cases were recurred and the recurrence rate was 7.14% .In routine group, 19 cases were cured,7 cases were mended,2 cases were invalidated and the rate of cure was 70.00% .The cured 21 cases were followed up for one year,7 cases were recurred and the recurrence rate was 33.33 % . Conclusion: The advantage of peritectomy curing Mooren s corneal ulcer is that the period of treatment is shorter,curative effect is better and the recurrence rate is lower.
