目的 通过观察药费对高血压病人治疗的影响力度 ,来探讨价效医学在高血压治疗中的意义。方法 从2 0 0 0年 7月起 ,对 85例需要药物治疗的高血压病人 ,发给健康教育处方 ,使病人及家属了解高血压的危害 ,一般常识及坚持治疗的重要性和药费情况。分为自愿选择高药费组 (A组 ) ,低药费组 (B组 )和强行高药费组 (C组 )治疗方案。并追踪坚持治疗及血压控制情况。结果 多数 (6 1 7% )病人选择低药费治疗方案 ,且有较高的坚持服药率 6 9 2 % ,高药费组坚持服药率 5 7 5 % ,强行高药费组坚持服药率最低 4 4 %。因此 ,廉价药治疗 ,总效益明显增大。结论 多数病人愿意接受低药费治疗方案 ,采用低费用药品 ,可以提高治疗率 ,可使总体效益提高。临床应从价 -效医学观念出发 。

Objectives To investigate the effects of payment in hypertensive therapy by patient selecting the higher or lower fee. Methods Since July 2000 to June 30 th , 2001, 86 hospitalized patients had been given health-education prescriptions to let them and their families know the dangerousness of hypertension, the importance of insisting therapy and how much about medicine fee. The patients who chose higher payment by themselves belonged to group A, and lower payment to group B, meanwhile, the other forced higher payment to group C. Results The most patients (61 7%) had selected the lower payment, their rate of insisting therapy was 64 1%, but the rate of the higher payment group B was 57 5%, and the group C was 44%. The medicine had lower cost was easier acceptable for the most patients. Conclusions Most patients preferred lower payment therapy of hypertension. The medicine with lower cost could increase the rate of hypertensive therapy and control, the doctors should use the price-effect medicine concept in their clinical works, and choose the best way according condition and economy of patients for deciding therapeutic scheme.
