青光眼并白内障联合手术不同术式的探讨

A clinical analysis of different operation methods to treat glaucoma accompanied with cataract.

目的 探讨青光眼白内障联合手术中不同术式的适应证及手术效果。方法 对本院 1995年来 45例青光眼并白内障病例进行手术适应证分析及疗效分析。结果 小梁切除加白内障超声乳化人工晶状体植入术 ,并发症少 ,视力提高显著 ,眼压控制稳定。小梁切除加白内障囊外摘出人工晶状体植入术并发症比前者多 ,视力不同程度提高 ,眼压控制稳定。白内障囊外摘出或超声乳化人工晶状体植入术加虹膜周边切除术术后疗效好。结论 小梁切除加超乳术式为联合手术中最好的方法 ,但适应证有局限性 ;小梁切除加囊外摘出术式适应证广 ,并发症比前者多 ,但能尽量克服 ;摘出加周切术式组织伤小 ,手术时间短 ,但也有局限性。

Objective To evaluate the effect of different operation methods to treat glaucoma accompanied with cataract.Methods An retrospective study was underwent to analyze the indication and different surgery effect of 45 cases of glaucoma accompanied with cataract.Results The patients who were underwent trabeculectomy and phacoemulsification got better visual acuity,better intracocular pressure control and few surgery complications than the cases who underwent trabeculectomy and extracapsular cataract extraction.The cases that underwent extracapsular cataract extraction or phacoemulsification combined with peripheral iridectomy also got good results.Conclusion Trabeculectomy and phacoemulsification is the best choice.But it also has some limitations.The indication of trabeculotomy and extraction cataract extracapsular is broad.But it can easily arose many complications.We should try to avoid it.Cataract extracapsular or phacoemulsification combined with peripheral iridectomy have slight damage to the tissue and short operation duration,but it also have its limitations.
