白内障超声乳化手术中后囊破裂的预防

Prevention of posterior capsular rupture in phacoemulsification.

目的 探讨白内障超声乳化手术中后囊破裂的预防措施。方法 对 2 86例 3 18眼超声乳化手术中发生的 2 8眼后囊破裂原因进行分析。结果 3 18眼内白内障超乳化手术中2 8眼后囊破裂 (8 8% )。前 80例发生率最高达 14例 (17 5 % )。随着熟练程度不断提高发生率相对减少 ,手术至 15 0眼时 2 2例(14 66% )。后期 168例仅 6例 (3 5 % )。后囊破裂的并发症有玻璃体溢出 8例 ,硬核碎片坠入后玻璃体腔 2例 ,其中 1例 1月后发生视网膜脱离。结论 后囊破裂的发生率与手术医生的操作熟练程度呈负相关。

Objective To investigate preventive measures of posterior capsular rupture in phacoemulsification.Methods The causes of posterior capsular rupture in 28 of 318 eyes (286 cases) having undergone phacoemulsification were analyzed.Results Intraoperatively, posterior capsular rupture occurred in 28 eyes (8.8%). In the first 80 eyes it occurred in 14 eyes (17.5%) and was the highest occurrance rate.In the last 168 eyes there were only 6 eyes (3.5%).The commom complications were vitreous spilled over 8 eyes, drop of fragment into posterior vitreous 3 eyes, and retinal detachment in l of them after one month Conclusion The occurrance rate of posterior capsular rupture is inversely proportional to that of operation skill.
