治疗性穿透性角膜移植术中偏中央植片的应用

Effect of eccentric therapeutic penetrating corneal transplantation.

目的 研究治疗性穿透性角膜移植术中偏中央植片的必要性和减少角膜散光改善视功能的方法。方法 记录植孔中心与角膜中心连线延长线上植孔外缘与角膜缘的距离 ,取供眼相同位置的角膜植片移植。对 18例角膜溃疡穿孔偏位者施行偏中央植片的穿透性角膜移植术 ,平均随访 12月。结果 13例移植片透明 ,10例裸眼视力 0 1以上。结论 治疗性穿透性角膜移植术中采用偏中央植片定向标记技术效果可靠 ,可减少角膜散光 。

Objective To study the necessity of eccentric therapeutic penetrating corneal transplantation to restore sight and control the psotoperative corneal astigmatism. Methods Marks were made on the link betweenteh center of recipient bed and the corneal center, the distance between the external edge of recipient bed and the limbus was also made. Then the donor graft was taken out from a donor eye in the corresponding position and sutured on the recipient bed. 18 cases with eccentric corneal ulcer and perforation were performed and followed up for 12 months. Results The grafts of 13 cases kept transparent. The visual acuities were obtained above 0.1 in 10 cases. Conclusion The orientation marking technique can keep grafts transparent and reduce the postoperative corneal astigmatism and improve visual acuity in eccentric penetrating keratoplasty.
