摘要
目的 探讨CD44V6和MMP -2在鼻咽癌 (NPC)组织中的表达情况及其与NPC浸润转移的关系。方法 收集 2 2 9例NPC存档保存完好的石蜡病理标本 ,采用免疫组织化学染色技术 ,检测CD44V6和MMP -2在正常鼻咽和NPC组织中的表达情况 ,分析CD44V6和MMP -2与NPC浸润转移的关系。结果 CD44V6和MMP -2在鼻咽炎症组织中均呈阴性表达 ;在NPC组织中CD44V6为高阳性表达 ,总表达率 83 0 %。但在NPC各TNM分期中 ,包括有颈淋巴结转移与无淋巴结转移、有远处转移与无远处转移 ,CD44V6阳性表达率差异均无显著性 (P >0 .0 5 ) ;MMP -2总阳性表达率为 5 2 .0 % ,其中 ,T3~ 4高于T1~ 2 (P <0 .0 1) ,有淋巴结转移或远处转移者高于无淋巴结转移者 (P <0 .0 1) ,N2~ 3 高于N1(P<0 .0 5 ) ,N2 ,N3 与M1相当 (P >0 .0 5 ) ;而MMP -2和CD44V6在NPC的表达并无密切关系 (P >0 .0 5 )。结论 CD44V6阳性表达对NPC诊断有辅助意义 ,但与NPC浸润转移程度无明显相关 ,对NPC侵袭转移预测意义不大 ;MMP -2与NPC浸润转移及其严重程度密切相关 ,值得进一步跟踪研究 ,可望成为预测NPC临床生物学行为特征的有用标记物。
Objective To study the expressions of CD44V6 and MMP-2 in nasopharyngeal carcinoma(NPC) and the relationships between expression of CD44V6 or MMP-2 and invasions or metastasis. Methods By routine immunohistochemical staining, the expressions of CD44V6 and MMP-2 in 229 cases of NPC and 20 cases with nasopharyngeal inflammation were studied. The relationship between positive expression of CD44V6 and MMP-2 in NPC and invasions of metastasis was analyzed. Results In all patients with nasopharyngeal inflammation, the expressions of CD44V6 and MMP-2 were negative. But in patients with NPC, the expression rate of CD44V6 was as high as 83.0% . However, the differences in the positive rates of CD44V6 were not statistically significant ( P > 0.05 ) among all TNM staging of NPC. The positive expression rate of MMP-2 in NPC was 52.0% . And the positive rate of T 3~4 was higher than that of T 1~2 ( P < 0.01 ), and was higher in lymph node metastasis or distant metastasis ( P < 0.01 ). The positive rate of N 2~3 was higher than N 1 ( P < 0.05 ), and N 2, or N 3 was similar to M 1 ( P > 0.05 ). Conclusion The expression rate of CD44V6 is considerably high in NPC. It may help clinical diagnosis of NPC. But there is no correlation between the positive expression of CD44V6 and the degree of invasion or metastasis. The expression rate of MMP-2 in NPC is also high. And there is significant correlation between the expression level of MMP-2 and the degree of invasion or metastasis. MMP-2 may be a useful marker in predicting the biologic characteristics of NPC.
