膝关节镜下处理半月板损伤的疗效观察

THERAPEUTIC OBSERVATION ON THE ARTHROSCOPIC TREATMENT OF MENISCUS INJURY

目的 探讨应用膝关节镜技术处理不同类型的半月板损伤的临床疗效及影响疗效因素。方法 对 6 7例 76侧半月板损伤的病人应用现代微创技术 (膝关节镜技术 )行系统镜下检查 ,确定有半月板前角损伤 12例 ,体部损伤 37例 ,后角损伤 2 7例 ,对 4 9侧半月板行部分切除术 ,对 11侧行次全切除术 ,全部切除 14侧以及缝合 2侧 ,同时对合并病变如骨赘、滑膜皱襞及交叉韧带损伤予以处理。结果 满意 4 0例 (5 9.7% ) ,良好 15例 (2 2 .4 % ) ,一般 6例 (8.9% ) ,差 6例 (8.9% ) ,总有效率 91.1%。结论 应用膝关节镜技术处理半月板损伤有创伤小、疗效好、术后关节功能恢复快以及病人易接受等优点 。

Objective To explore the therapeutic effect of arthroscopic treatment on different types of meniscus injuries and to discover the correlative factors.Methods 12 cases of anterior injury,37 cases of somatic injury and 27 cases of posterior injury were confirmed through arthroscope.49 cases underwent partial excision,11 subtotal resection and 14 total removal.Complications such as osteophyte,synovial folds and injury of cruciate ligaments were treated simultaneously. Results The total effective rate was 91%,satisfactory recovery was got in 40 cases (59.7%),good recovery in 15 cases (22.4%),ordinary in 6 cases (8.9%) and poor in other 6 cases (8.9%) .Conclusion Arthroscopic treatment of meniscus injury has proved to be an ideal approach for its no arthratomy,minor trauma,good effectiveness and quick functional recovery.
