胸水嗜酸粒细胞增多症23例临床分析

Clinical analysis of pleural fluid eosinophilia of 23 cases

目的 探讨胸水嗜酸粒细胞增多症 (PFE)的原发病、临床表现、治疗及预后。方法 回顾分析 2 3例PFE的临床特征。结果 2 3例胸水均为渗出性 ,4例伴血嗜酸粒细胞增多 ;10例在病程中伴发气胸 ;原发病中恶性胸腔积液占 2 6 1% (6 /2 3) ,同期非PFE者恶性肿瘤占 32 6 % (5 8/178) ,PFE与非PFE渗出性胸水的恶性肿瘤发生率比较无显著性差异 ;结核性胸膜炎占 2 1 7% ,其中初次胸穿即为PFE者仅 1例。结论 PFE原发病以气胸、恶性肿瘤和结核为多 ;它的出现并不能肯定为良性疾病。

Objective To analyse the primary affection,clinical manifestation,treatment and prognosis of pleural fluid eosinophilia(PFE).Methods 23 patitents with PFE were analysed tetrospectively.Result All of 23 patients pleueal fluid were exudative,4 patients with blood eosinophilia.10 patients were with pneumothorax in the course.The other primary affections included malignant effusion in 6 patients(26.1%),tuberculosis in 5 patients(21.7).At the same time 58(32.6%) malignant effusions were found in 178 noneosinophilic effusions,there was no significant difference in the prevalence between eosinophilic and noneosinophilic effusions according to malignant effusion.Only one patient with tuberculosis was found PFE at the initial thoracentesis.Conclusion The more often primary affection of PFE were pneumothorax,malignant tumor and tuberculosis.PFE cannot be considered as a predictor of an underlying benign disorder.
