核磁共振信号改变对脊髓型颈椎病保守治疗效果的预测 被引量:5

摘要 目的探讨核磁共振(MRI)信号改变能否预测脊髓型颈椎病(CSM)保守治疗的效果。方法总结87例轻型CSM病人保守治疗随访结果与颈脊髓T2加权信号增强(ISI)的关系。结果治疗前功能评分28.5±2.5(26~35),最后随访32.5±2.5(28~37),评分增加4.6±1.7(-4~10);多节段ISI、局限性ISI及无ISI3组病人年龄、发病时间、治疗前后评分及治疗改善率、满意率均无明显差异;ISI减弱、无变化或无ISI均不影响保守治疗效果。结论对轻型脊髓型颈椎病,出现脊髓信号增强病人通过保守治疗也能有较好预后。 Objective To investigate whether increased signal intensity(ISI) of spinal cord could predict the outcome of conservative treatment.Method Eighty-seven patients with mild CSM underwent conservative treatment. The outcome was assessed and compared with T2-weighted images which showed ISI.Result The average neurologic assessing score was 28.5±2.5(range, 26~35)before conservative treatment and 32.5±2.5(range,28~37)at final follow-up. No statistically significant difference was seen among three groups of patients without ISI, with focal ISI and with multisegmental in satisfactory rate, assessing scores before and after treatment and age of patients, suffering age.Conclusion Patients with increased signal intensity of spinal cord will also have good prognosis in patients with mild CSM.
