椎基底动脉供血不足患者脑干功能损害与经颅多普勒检测的相关性

The correlation of brainstem function injuries and transcranial Doppler on the vertebrobasilar ischemia

目的探讨椎基底动脉供血不足(VBI)患者脑干功能与经颅多普勒(TCD)检测的相关性。方法对68例临床诊断(VBI)的患者进行瞬目反射(BR)、脑干听觉诱发电位(BAEP)和TCD检测。结果VBI组脑干功能损害84.6%,TCD异常率为83.1%,χ2检验差异无显著意义;非VBI组脑干功能损害8.3%,TCD异常率为46.6%,χ2检验差异有显著意义。VBI组脑干功能损害及TCD异常者为98/136,皆正常者为24/136,两者占89.7%;非VBI皆异常者为4/60,皆正常者为32/60,两者占60%。结论根据临床表现,结合BR、BAEP及TCD检查结果可提高对VBI的诊断价值,而单独的TCD结果不足以作为诊断依据。

Objective To investigate the correlation of br ainstem function injuries and transcranial Doppler(TCD)examination on the vertebrobasilar ischemia(VBI ).Methods 68patients with VBI and 30patients with non-VBI were studied by blink reflex(BR),brainstem au-ditory evoked potentials(BAEP)and TCD .Results It showed that the incidence of brain stem function injuries in VBI group was 84.6%,TCD83.1%,there being no significant di fference by Chi-square test,while i n non-VBI group brainstem function i njuries 8.3%,TCD 46.6%.Both brainstem function injuries and TCD were abnormal in VBI 98/136,non-VBI 5/60and normal VBI 24/136and non-VBI 32/60.Conclusions It indicates that BR,BAEP combined with TCD and clinical signs can make th e diagnosis of VBI.The results of TCD alone can not be a considerable diagnostic basis for VBI.
