高血压左室肥厚与QT离散度的关系及阿替洛尔对其影响

摘要 目的研究高血压左室肥厚与QT离散度的关系及阿替洛尔对其影响.方法 102例高血压病患者通过超声心动图划分为左室肥厚组68例,无左室肥厚组34例,与30例健康对照组进行QTd,QTlcd测定.随机选出50例行常规降压治疗,另52例在常规治疗基础上加用阿替洛尔,血压降至理想水平4周后复测QTd,QTlcd.结果 1)高血压左室肥厚患者的QTd明显高于非左室肥厚及健康者;2)高血压病左室肥厚程度与QT离散度呈正相关;3)阿替洛尔治疗后QT离散度明显缩小;4)阿替洛尔治疗4周后心肌肥厚无明显逆转.结论高血压左室肥厚与QT离散度增加有关;阿替洛尔可明显缩短高血压左室肥厚患者的QT离散度,而减少心脏病事件发生率,长期应用可能因逆转左室肥厚而效果更佳. objective To study the relation between left ventricular hypertrophy and QT interval dispersion (QTd) in essential hypertension and the effect of Atenolol on QTd.Methods 102 hypertension cases were divided into the left ventricular hypertrophy(LVH) group (68 cases) and Non-left ventricular hypertrophy(non-LVH) group (34 cases) according to echocardiogram. 50 cases were chosen at random and received routine anti-hypertension treatment, while the other 52 cases were treated with Atenolol on the basis of the routine anti-hypertension treatment.Contrasting with 30 healthy persons,all the persons were examined QTd and QTlcd after 4 weeks.Results 1)The QTd in hypertension with left ventricular is remarkably longer than that of non-left ventricular hypertrophy and healthy person.2)LVH degree of hypertension is positively correlated with QTd;3)QTd decreased remarkably after treatment with Atenolol;4)Left ventricular hypertrophy don't reverse after treatment with Atenolol for four weeks.Conclusions Left ventricular hypertrophy in hypertension is related to QTd prolonging;Atenolol can diminish the QTd in hypertension significantly, and then decrease the incidence of cardiovascular affairs.It will obtain more optimistic effect for long-term application.
