颅骨-锁骨发育不全综合征在曲面体层片上的影像特点

Cleidocranial dysplasia:identifying features on panoramic radiography

目的 分析颅骨 -锁骨发育不全综合征在曲面体层片上的特征性表现。方法 颅骨 -锁骨发育不全综合征 18例 ,在曲面体层片上观察牙、颌骨及颧弓等发育情况。对 7例进行了随访观察。结果 ①牙发育异常 :乳牙滞留 (14 / 18)、恒牙迟萌 (16 / 18)、牙发育畸形 (11/ 18)、多生牙及阻生牙 (各 18/ 18)、含牙囊肿形成 (5 / 18)及继生多生牙 (4/ 7) ;②颌面骨发育异常 :牙槽骨致密 (9/ 18)、骨小梁粗糙 (18/ 18)、升支前后缘呈平行 (14 / 18)、喙突尖小(12 / 18)及颧弓下弯畸形 (18/ 18)。结论 ①建议用骨 -牙发育异常综合征命名较妥 ;②只需一张曲面体层片即可对本病作出正确诊断 ;③因本病进行正畸治疗应定期曲面体层片随访 ,以便早发现继生多生牙 ,早处理。

Objective To analyse the identifying features presented on panoramic tomography of cleidocranial dysplasia.Methods Eighteen cases of cleidocranial dysplasia were collected to analyse the developments of jaws,zygomatic arch,tooth on panoramic tomographs.Seven of them were followed up.Results ①Developmental abnormalities of teeth:retention of deciduous teeth (14/18),delayed eruption of permanent teeth (16/18),developmental abnormal teeth (11/18),supernumerary teeth and impacted permanent teeth (18/18 respectively),dentigerous cyst formation (5/18) ,and continuous supernumerary teeth emerging (CSNTE)(4/7);②Developmental abnormalities of bone in oral and maxillofacial region:increased density of alveolar crestal bone (9/18),coarse trabeculation of mandible (18/18),narrow ascending ramus with near paralleling sided anterior and posterior borders (14/18),slender and pointed coronoid process (12/18) and severe downward zygomatic arch (18/18).Conclusion ①To suggest the use of osteodental developmental abnormalities syndrome as the name for this disease;②A diagnosis of cleidocranial dysplasia can be made based on only a panoramic radiograph;③Panoramic tomography should be taken periodically during the period of orthodontic treatment so that the CSNTE can be found and treated early in order to maintain the orthodontic stability.
