福建省新生儿足跟血促甲状腺激素水平分析

目的 了解福建省特需人群 (育龄及孕产妇女 )的碘营养水平。方法 新生儿出生后 72 h采足跟血 ,滴在规定的滤纸上。采用时间分辨荧光免疫测量法 (DEL FIA法 )测定促甲状腺激素 (TSH)浓度。 1998— 2 0 0 0年共测定福建省 9个地区 5 6家医院正常产新生儿足跟血 916 77例。结果 福建省新生儿 TSH中位数为 2 .82m U / L ,TSH值 >5 m U / L者占 14 .85 %。 3年间差异无显著意义 (F =1.88,P >0 .0 5 ) ,但地区间差异存在显著意义 (F =38.38,P <0 .0 1)。结论 该人群仍存在缺碘问题 ,对孕妇和 0～ 2岁婴幼儿的碘营养监测和补碘工作仍需加强。建议在新生儿足跟血 TSH>5 m U/ L的比率 <3%的基础上建立脐带血

Objective To find out the iodine nutrition levels of special population (such as child bearing aged, pregnat and lying in women)in Fujian province.Methods Sample of heel blood collected from newborn after 72 hours, and dropped on stipulated filter paper to determine the TSH concentration of blood according to DELFIA. The heel blood samples of 91677 newborns by nomal birth were tested in 56 hospitals of 9 areas in Fujian.Results Among the 91677 newbrons,there are 14.85% with TSH over 5 mU/L. The percentage of newborns' TSH greater than 5 mU/L in 1998-2000 is not significantly difference (F=1.88,P>0.05),but areas' level are significantly difference (F=38.38,P<0.01).Conclusions Iodine deficiency disorder problem still exists among these population,iodine nutritional surveillance and replenish iodine for pregnant women and infants aged 0~2 years still need to be strengthened.TSH cut off point of neonatal cord blood is bases on that the frequency of neonatal TSH above 5 mU/L heel blood is less than 3%.
