摘要
目的 :探讨加膜食管支架对食管癌并发食管 气管瘘的实用价值。方法 :X线引导下为 18例患者置入 18个加膜支架 ,并在其中 6个支架加膜部分的外周包裹了一薄层明胶海绵 ,以防支架与食管壁间形成不规则腔隙。同时对置入装置进行了改良。结果 :支架置入顺利 ,术中无严重并发症发生。术后呛咳即刻消失 ,吞咽困难明显缓解 ,由术前平均3 .4级降至 0 .9级。结论 :加膜支架治疗食管癌并发食管 气管瘘安全有效 。
Objective: To evaluate the covered stents in the treatment of esophagorespiratory fistula associated with esophageal carcinoma. Methods: 18 covered sents were implanted under fluoroscopic guidance in 18 patients. In 6 of the 18 stents a thin layer of gelfoam was added surrounding the covered stent to avoid formation of an irregular space between the stent and esophagus. Results: The stent implantation was successful and no serious stent related complications happened to all cases. After stenting, the fistulas occluded and symptoms of aspiration were relieved immediatly,the average degree of dysphagia lowered from grade 3.4 to 0.9. Conclusion:The implantation of covered stents is a safe and effective method for the treatment of esophagorespiratory fistula associated with esophageal carcinoma.Our modified method of implantation effectively avoid the formation of an irregular space between the stent and esophagus.
