鼻腔鼻窦内翻性乳头状瘤切除术及综合治疗

Combined Therapy for Inverted Papilloma of Sinonase

目的 探讨减少鼻腔鼻窦内翻性乳头状瘤术后复发的治疗方法。 方法 33例鼻腔鼻窦内翻性乳头状瘤患者采用面中部翻掀进路及鼻侧切开进路切除肿瘤 ,配合激光、干扰素肌注和局部涂鸦胆子油等方法治疗。 结果 经综合治疗后 ,33例患者全部治愈。随访 1～ 9年 ,19例术后 3个月内发现鼻腔有小肉芽 ,有 7例病理活检示乳头状瘤残留或复发 ,经 1～ 2次 Nd∶ YAG激光光灼均治愈。 结论 综合治疗对减少鼻腔鼻窦内翻性乳头状瘤术后复发具有重要意义。

Objective To study an effective treatment for diminishing postoperative recurrence of inverted papilloma in sinonase. Methods 33 patients were treated by midfacial degloving approach resection or lateral rhinotomy and cooperated with laser, interferon and Brucea javanica after operation. Results All 33 patients were cured by combined therapy. Follow up period was 1~9 years. Mini granulation had been found in 19 cases within three months after operation. 7 of them were demonstrated to be residual or recurrent tumor postoperatively by biopsy, and all were cured by one or two times Nd∶YAG laser treatment. Conclusion Combined therapy of inverted papilloma in sinonase may play an important role in preventing recurrence postoperatively.
