摘要 随着神经影像学的进步,未破裂动脉瘤(unruptured intracranial aneurysms,UIAs)的发现率也相应提高.由于动脉瘤性蛛网膜下腔出血(subarachnoid hemorrhage,SAH) 30 d的死亡率为45%,大约半数生存者均有不可逆的脑损害[1],这就给神经外科医生提出一个问题,即如何对待这种目前无症状但可能发生危险的病变.预防出血是降低死亡率的最有效方式,因此有理由考虑对UIAs在破裂前进行治疗.然而,手术治疗和栓塞治疗并非没有危险.目前UIAs患者是应该保守治疗还是应该尽早进行外科处理尚存在着争论.
