摘要
目的 研究老年肾衰血透 (HD)的临床特点 ,探讨如何提高老年患者HD的生存率和生活质量。方法 对比分析 1 999年 3月— 2 0 0 1年 3月 40例老年HD及 30例非老年HD的临床资料。结果 老年HD患者继发性肾损害 (糖尿病肾病 ,高血压肾病 )为35 % ,较非老年HD组为高 (P <0 .0 5) ;老年组心血管并发症及感染较非老年组高 (P <0 .0 5) ,营养状态较非老年组为差 (P <0 .0 1 ) ,透析充分性较非老年组为差 (P <0 .0 1 ) ;心衰、心律失常、营养不良及感染是老年HD死亡的主要原因。结论 早透析 ,改善营养不良 ,预防感染 。
Objective To study the clinical characteristics of the elderly HD patients and to investigate how to improve their survival rate as well as their quality of life. Methods Contrastive study and analysis were performed about the clinical data of 40 elderly HD patients and 30 young HD patients during March 1999 and March 2000. Results Secondary kidney lesions (diabetes and hypertensive nephropathy) were found higher in the elderly group (35%) than in the control group (P<0.05); cardiovascular complications and infections were discovered higher in the elderly group than in the control group (P<0.05); nutrition status and dialysis capacity were both found lower in the elderly group. Heart failure, arrhythmia, malnutrition and infection were the main causes for the death of the elderly HD patients. Conclusion Early performance of dialysis, improvement of nutrition, prevention of infection and control of cardiovascular complication are all the necessary means to improve the survival rate and quality of life for the elderly HD patients.
