白内障超声乳化术中后囊破裂的后房型人工晶体植入术

Posterior Chamber Intraocular Lens Implantation inPosterior Capsular Rupture during Cataract Phacoemulsification

目的:评价白内障超声乳化术中后囊破裂的Ⅰ期后房型人工晶体植入术的疗效。方法对 ２３１例(２６９眼)施行白内障超声乳化术中后囊破裂 ３９例(４０眼)行Ⅰ期后房型人工晶体植入术。结果:囊袋内植入 １２眼;人工晶体先学部前囊固定 １３眼,睫状沟内植入 １３眼,２眼需缝线固定。随防 ｌ－３７个月,裸眼视力≥０．５者 ３３眼(８２．５%),≥１．０者 １９眼(４７ ５%);矫正视力≥０．５者 ３８眼(９５%),≥１．０者 ２５眼(６２．５%);并发症主要有:人工晶体偏位３眼、瞳孔变形２眼。结论:白内障超声乳化术中后囊破裂时,控制玻口的扩大,利用前、后囊作支撑,结合前段玻璃体切割术,植入后房型人工晶体仍可取得满意的效果。

Objective: To evaluate the effect of posterior chamber intraocular lens (IOL) implantation in posterior capsular rupture during Cataract Phacoemulsification. Method: Posterior chamber IOL implantation was performed in 39 cases (40 eyes) of posterior capsular rupture during cataract phacoemulsification. Results: In-the-bag implantation was performed in 12 eyes, optic-IOL was fixed on anterior capsular in 13 eyes, sulcus-fixed lens implantation in 13 eyes, two of them requiring suture fixation. One to 37 months' follow-up after the operation revealed that the postoperative uncorrected visual acuity was 0. 5 or better in 33 eyes (82.5% ) and 1.0 or better in 19 eyes (47. 5% ). The corrected visual acuity was 0. 5 or better in 38 eyes (95% ) and 1.0 or better in 25 eyes (62. 5% ) . The major complications were IOL deviation ( 3 eyes) and pupil transformation ( 2 eyes). Conclusion: Posterior chamber IOL implantation can be performed with satisfactory result in case of posterior capsular rupture during cataract phacoemulsification.
