十二指肠乳头旁憩室与胆道结石关系的探讨(附37例分析)

Study on the Relationship Between Juxtapapillary Duodenal Diverticulum and Cholelithiais (Analysis of 37 Cases)

目的 :探讨十二指肠乳头旁憩室与胆系结石的关系。材料与方法 :通过对 5 6例十二指肠憩室的ERCP和X线摄片资料的归纳和分析 ,找出相互关系。结果 :位于乳头旁的十二指肠憩室对胆道的挤压致使胆道扩张和乳头功能受损 ,胆汁淤积 ,细菌感染 ,胆汁中的胆红素葡萄糖苷酸被水解为未结合的胆红素与钙盐结合、沉积而形成胆系结石。结论 :憩室越大 ,距乳头越近 ,越易发生胆道疾病。其对胆道的影响大于对胰腺的影响。

Purpose:To study the relationship between juxtapapillary duodenal diverticulum and cholelithiasis.Materials and Methods:Materials of 56 cases with doudenal diverticulum examined by X-ray and ERCP were analyzed and found their correlation.Results:Biliaries are extruded by juxtapapilla duodenal diverticulum.It makes that biliaries are dilated,the function of papilla is injuryed.Bile were silted up and infeeded with germs.Bilirubin glucuronide of bile is hydrolyzed into muncombined bilirubin and it is combined with calcium and depositsm,then forms cholelithiasis.Conclusion:More bigger is diverticulum and more nearer is it away from papilla,more easier dose disease of biliary occur.it effects disease of biliary more than pancreas.
