产超广谱β-内酰胺酶细菌医院感染危险因素的主成分及逐步回归分析

PRINCIPAL COMPONENT ANALYSIS AND STEPWISE REGRESSION ANALYSIS OF RISK FACTORS IN NOSOCOMIAL INFECTION CAUSED BY EXTENDED-SPECTRUM β-LACTAMASES-PRODUCING BACTERIA

目的 :研究产超广谱 β -内酰胺酶 (ESBLs)细菌医院感染的危险因素。方法 :用主成分分析和逐步回归分析浙江省 6所医院 185例ESBLs阳性细菌医院感染病例和 185例ESBLs阴性细菌医院感染对照。结果 :主成分分析从 14个危险因素中提取了 5个主成分 ,其方差累积贡献率达 6 0 .2 %。逐步回归分析揭示 3个主成分有显著性意义 ,分别为 :侵袭性医疗操作和三代头孢菌素应用 ;住院时间和抗生素的应用 ;导管检查。结论 :产ESBLs细菌医院感染为多因素所致 ,主要与医院的侵袭性操作及抗生素的使用有关。加强消毒灭菌 。

Objective:To determine the risk factors of nosocomial infection casued by Extended-spectrum β-lactamases (ESBLs)-producing bacteria. Method:One hundred and Eighty_five cases of noscoomical infection with positive-ESBLs bascteria and 185 cases control with negative-ESBLs bacteria were investigated by principal component analysis and principal component stepwise regression analysis between May 1999 and May 2000 in six hospitals of Zhejiang province.Results:Five principal components were extracted from 14 risk factors of nosocomial infection caused by Extended-spectrum β- lactamases (ESBLs)-producing bacteria through principal component alaysis,and the cumulative contribution of variances of the 5 principal components was 60.2%.The results of stepwise regression analysis revealed that three principal components had significant difference.They were ranked as follows:invasive medicinal operation and third generation cephalosporins administration;time of hospitalization and application of antibiotics;catheterization.Conclusions:The nosocomial infection of Extended-spectrum β-lactamases (ESBLs)-producing bacteria is casused by multiple factors,and mainly related to invasive operation and application of antibiotics.Augmenting disinfection and sterilization,controlling the usage of antibiotics is conducive to reducing the occurrence of the nosocomial infection caused by Extended-spectrum β-lactamases(ESBLs)-producing bacteria.
