高频心电图和心室晚电位与心电图异常的临床研究

DETECTION OF HIGH RISK SIGNALS IN VENTRICULAR LATENT POTENTIAL AND HIGH FREQUENCY ECG AND THEIR CLINICAL IMPLICATION

摘要 目的 :探讨高频心电图 (HFECG)和心室晚电位 (VLP)对恶性室性心律失常、心脏性猝死的预测价值。方法 :分析 781例各类心血管病人的ECG ,HFECG和VLP ,并将异常信号区分为高风险和非高风险两大类。结果 :高风险异常信号的检出率ECG为 78% ,VLP为 1 3 % ,HFECG为 52 %。结论 :由梗塞 /缺血心肌病灶产生的ECG高频信号的高检出率并不能说明其具有高预测价格。计算机辅助的VLP和HFECG反映了这种碎裂延迟电位高频成份有临床应用的潜在价值 。 Objective:To explore the positive and negative prediction value for malignant ventricular arrhythmia and sudden death of the computer assisted High Frequency Electrocardiography (HFECG) record and Ventricular Latent Potential (VLP) in cardiovascular diseases. Methods:We recorded and analyzed the routine ECG,HFECG and VLP of 781 patients with various cardiovascular diseases to stratify the high risk and non-high risk abnormal signals. Result:The high-risk signal detection rate of ECG was 78%,VLP was 13%, and HFECG was 52%. Conclusions:Since the routine ECG records lack the high frequency components that were generated from the ischemia/infarction foci of the myocardium, the apparently high detection rate does not translate to prediction value. The computer science adopted VLP and HFECG reflect the high frequency components are potentially useful in clinical pratice.The standardization needs further improvement.
