超声诊断在腹腔镜胆囊切除术前的应用价值

Application of B-Ultrasonic Examination in Laparoscopic Cholecystomy

目的 探讨超声在腹腔镜胆囊切除术 (LC)术前的应用价值 .并与内窥镜逆行性胆管造影 (ERC)对比研究 .方法 对 5 6例可疑肝外胆管病变患者 ,应用超声观察胆管脂肪餐前后变化情况 ,同时行ERC检查 .结果 与手术对照 ,5 6例可疑肝外胆管病变者脂餐后胆总管增宽的有 38例 ,其中 2 9例为胆管疾病 ,而脂餐后胆总管明显缩小的病例 ,未见胆管疾病 .结论 超声检查中的脂肪餐试验对筛选胆管病变有较高的特异性和敏感性 。

Objective To explore the application value of ultrasongraph in laparoscopic cholecystomy, combine with endoscopic retrograde cholangiography. Methods Application of B-ultrasonic examination in mildly dilated bile duct after fatty meal on 56 cases with bile duct disease,and comparel with endoscopic retrograde cholangiography. Results Out of 38 cases whose inner diameters expanded after a fatty meal,29 cases suffered from bile duct disease. If the inner diameters are obviously reduced after a fatty meal,those are not the bile duct disease. Combime operative, the sensitivity were 100% of B-ultrasonic examination, and the specificity were 90.3%, the positive predictive values was 84.5%, the negative predictive values was 100%. Conclusions B-ultrasonic examination in mildly dilated bile duct after fatty meal has a high specificity and sensitivity, and can avoid the part( 48.2 %) to get check in endoscopic retrograde cholangiography.
