摘要
目的 MRI引导下激光导热疗法 (LITT)处理动物正常肝脏 ,判断病灶MRI实时信号反映病理结果的准确性。方法 新鲜猪肝 2 0个 ,按处理方式不同分为 2W 10min、4W 2 5min、4W5min、4W 7 5min、8W 2 5min 5个组 ,每组 4个。在开放私MR仪上 ,于射频扰相稳态采集反转梯度回波 (radiofrequencyspoiledsteadystateacquisitionwithrewoundgradientecho ,RSSG)序列下行LITT处理猪肝 ,监测病灶MRI信号变化至术后 5min ,对病灶大体直径和术后即刻 (术后 0min)及 5min时MRI信号直径进行比较 ,并观察MRI信号与病理结果的关系。结果 每组中LITT大体病灶与相应MRI信号直径比较采用配对t检验 :(1)LITT病灶大体直径与术后实时MRI信号直径比较 ,t值分别为 1 0 95、0 792、0 775、0 0 0 0、0 5 2 2 ,P值均 >0 0 5 ,统计学上差异无显著性意义 ,在病理上病灶大体和镜下表现与实时MRI信号存在着对应关系 ;(2 )LITT病灶大体直径与术后 5minMRI信号直径比较 ,t值分别为14 0 0 0、5 0 19、19 0 5 3、3 4 34、16 189,P值均 <0 0 5 ,统计学上差异有显著性意义。结论 MRI引导下LITT处理正常猪肝MRI实时信号可以反映实际病灶的变化。MRI引导下LITT可能成为治疗局部和区域性肿瘤的
Objective To deter mine the accuracy of real time MRI signal in detecting LITT lesions created in normal animal liver by using specific MRI pulse sequence guidance.Methods Twenty pork livers were divided into 5 groups with the thermotherapy intensity of 2 W 10 min, 4 W 2.5 min, 4 W 5 min, 4 W 7.5 min, 8 W 2.5 min, respectively. Under RSSG sequence guidance on open MRI scanner, real time MRI signals of LITT lesions created in normal pork liver were registered until 5 minutes after operation. The diameters of LITT lesions along the plane of MRI scanning were measured and HE staining of each lesion was performed. The gross and histological findings of LITT lesion were compared with the corresponding MRI signal, and the relation between MRI signal and pathological result was recorded. Results The diameters of LITT lesions were compared between the specimen and the corresponding MRI in each group: (1) Diameters of LITT lesions and corresponding real time MRI signals 0 minutes after operation were compared: t values were 1.095?0.792?0.775?0.000?0.522, respectively. Each P value>0.05, accordingly there was no statistical differences. And real time MRI signals accurately demonstrated the gross and histological findings of created lesions. (2)Whereas there was statistic differences between diameters of LITT lesions and corresponding MRI signals 5 minutes after operation, t values were 14.000?5.019?19.053?3.434?16.189, respectively and each P value<0.05. Conclusion Real time MRI signal can accurately represent the changes of LITT lesions in normal pork liver under MRI guidance. MRI guided LITT may provide a new and effective option for the treatment of local and regional tumor.
