门诊和住院病人抗高血压药物用药情况及依从性调查

Study of the Compliance of Antihypertensive Drugs in the Outpatients and Inpatients with Hypertension

目的 为探讨高血压住院和门诊患者降压药物的依从性及其相关因素 ;了解降压药物的用药情况 .方法 回顾性分析 1997年 12月至 2 0 0 1年 10月高血压病住院患者 4 0 8例和门诊随访患者 2 4 9例的降压药物依从性及其相关因素 ;调查降压药物的使用情况和药物副作用的发生率 ;观察药物依从者和不依从者的血压水平变化 .结果 高血压住院病人的心血管危险因素分层前几位依次为脂质代谢紊乱、吸烟、左室肥厚和糖尿病 .出院后 6 1.0 %的病人来院门诊随访 .住院期间的药物依从率为 6 3.5 %,出院时出院带药的依从率 77.1%,门诊随访期间的药物依从性为 4 3.4 %.结论 出院后对出院带药的依从性最佳 ,住院期间的药物依从性次之 ,门诊随访期间的药物依从性最差 .门诊随访期间钙拮抗剂和利尿剂的副作用明显增多 .药物依从者血压的波动小 ,药物不依从者血压的波动大 .

Objective To study the compliance of antihypertensive drugs and its related factors in the patients with hypertension including the inpatients and the outpatients.Methods The clinical data of 408 inpatients and 249 outpatients are reviewed to study the compliance of antihypertensive drugs and its related factors; to investigate the utilization of antihypertensive medicament and the incidence of the side-effect of the medication; and to explore the difference of the blood pressure control between the compliant group and the uncompliant group. Result The main cardiovescular risk factors of the hypertensive patients are in turn the disorder of lipometabolism,smoking,left ventricular hypertrophy(LVH) and Diabetes mellitus. And 61.0% inpatients followed up. The incidence of compliance is 63.5% in the inpatients, and 77.1% in the outpatients to the discharge order, and 43.4% in the following up outpatients.Conclusion The compliance of the medicament is best in the outpatient to the discharge order, and the compliance of the outpatients is worst.
