带旋髂深血管髂骨骨膜骨瓣治疗青壮年股骨头缺血性坏死

Vascular pedicled iliac periosteum and bone flap trans plantation for treatment of avascular necrosis of femoral head in young adults

目的 探索青壮年股骨头缺血性坏死的治疗方法临床效果。方法 采用带旋髂深血管髂骨骨膜骨瓣治疗青壮年股骨头缺血性坏死 36例 ,4 0髋。除 4例失访外 ,平均随访 2～ 8a ,并用股骨头缺血性坏死疗效百分法判定。结果 优 2 3例 ,良 7例 ,可 2例。优良率为 93.7%。结论 带旋髂深血管髂骨骨瓣可重建股骨头血运 ,重建骨小梁 。

Objective To investigate the method and clinical results to treat avascular necros is of femoral head(ANFH) in young adults.Methods 40 hips in 36 young adult patients with ANFH were treated using iliac periosteum and bone flap with pedicle of deep iliac circumflex vessels.32 cases were followed up for 2 to 8 years and the results were evaluated according to a new Grading Treatment System for ANFH recommended by the Chinese Society of Osteonecrosis.Results The therapeutic results in 23 cases were excellent,7 good and 2 fair.The rate for excellent and good results was 93.7%.Conclusions Transplantation of iliac periosteum and bone with vascular pedicle is effective in revascularization and new bone formation,which prevent the necrotic femoral head from progressive collapse.
