摘要
目的 探讨支气管哮喘患者气道粘膜上皮细胞是否表达基质金属蛋白酶 9(MMP 9)及其与气道嗜酸粒细胞浸润、气流受限的关系。方法 选取 10例支气管哮喘患者、6例COPD患者及 10例正常对照 ,进行纤维支气管镜粘膜活检 ,对活检组织做MMP 9免疫组化及HE染色 ;同时对患者进行肺功能测定及组胺激发实验。结果 所有支气管哮喘患者气道粘膜上皮MMP 9免疫组化均为阳性 ,2 / 6的COPD患者气道粘膜上皮MMP 9免疫组化为阳性 ,而所有正常对照气道粘膜上皮MMP 9免疫组化均为阴性 ;相关分析显示 :支气管哮喘患者气道粘膜上皮MMP 9免疫组化评分与气道嗜酸粒细胞计数呈正相关 (r =0 6 2 ,P <0 0 1) ,与FEV1呈负相关 (r =- 0 5 1,P<0 0 1) ,与PD2 0 FEV1不相关。结论 支气管哮喘患者气道粘膜上皮表达MMP 9,此可能为导致气道嗜酸粒细胞浸润及气流受限的重要原因。
Objective To investigate the expression of Matrix Metalloproteinase 9 (MMP 9) in bronchial biopy mucosal tissue and its relation to airway eosinophilic infiltration,lung function and airway hyperresponsiveness.Method 10 subjects with asthma were enrolled,meanwhile,10 healthy subjects and 6 subjects with COPD were as controls.At the same time,immunohistochemistry was employed.Results Immunohistochemistry for MMP 9 was positive in all asthmaticus,and just as such in 2/6 subjects with COPD.On the contrary,immunohistochemistry for MMP 9 was negative in all healthy subjects.The amount of MMP 9 expression was positively correlated with the number of eosinophil infiltrated to airway (r=0 62,P<0 01),negatively correlated with FEV1(r=-0 51,P<0 01),and not correlated with PD 20 FEV 1.Conclution MMP 9 which is expressed and secreted by human bronchial mucosal tissue is significantly correlated with eosinophilic infiltration,which might contributes to the reduction of lung function.
