Percutaneous Disc Decompression by Nd∶YAG laser: A Clinical Analysis

目的 探讨经皮Nd∶YAG激光腰椎间盘减压术的最佳适应证。方法 对 10 5例腰椎间盘突出症患者 14 4个椎间盘行经皮Nd∶YAG激光腰椎间盘减压术 (PLDD) ,术后随访 12个月 ,分析PLDD与患者年龄、病程及病变程度的关系 ,探讨最佳手术条件和适应证。结果 手术穿刺成功率 10 0 %。经过 12个月随访 ,总有效率 84 % ,其中年龄 <4 0岁患者的有效率为 93% ,术前病程 <2年患者的有效率为 88% ,椎间盘突出程度 <6mm者的有效率为 90 %。结论 经皮Nd∶YAG激光腰椎间盘减压术的最佳适应证为年龄 <4 0岁、病程 <2年、CT或MRI显示突出间盘

Objective To discuss the best surgical indications of percutaneous disc decompression by Nd∶YAG laser. Methods One hundred and five cases with 144 lesions of lumbar disc herniation were treated with percutaneous disc decompression by Nd∶YAG laser. The proper condifions for percutaneous laser disc decompression were analysed by patient's ages, course of disease and degree of lesion, which the surgical indictions and complications were evaluated. Results In this group of patients, the successful rate of penetration was 100%. The rate of efficiency was 93% in patients of less than 40 years old, 88% in thoes within two years course and 90% in patients whose lumbar disc herniation under 6 mm. The total rate of efficiency was 84% after 12 month follow up. Conclusions The percutaneous disc decompression by Nd∶YAG laser is best indicated to the patients of less than forty years old, within two years course of disease and under 6 mm disc in CT or MRI.
