树突状细胞在喉癌及癌前病变中的表达及意义

The role of dendritic cells mediated antitumor immunity in laryngeal cancer and precancerous lesions

目的 树突状细胞 (DCs)在喉癌及癌前病变中的表达及意义。方法 用免疫组化法检测 39例喉癌及癌前病变标本中DCs分布特点、DCs表面HLA DR的表达 ,以及肿瘤浸润性T淋巴细胞 (CD4 5RA、CD4 5RO表达 )免疫状态。结果 喉重度上皮不典型增生与喉鳞状细胞癌的DCs数量明显增多、呈簇状分布。前者T淋巴细胞CD4 5RA抗原低表达而CD4 5RO抗原高表达 ;喉癌组织中结果恰相反。结论 喉重度上皮不典型增生与喉鳞状细胞癌的DCs均增加 ,但喉癌DCs表面HLA -DR表达水平下降 ,与DCs免疫功能低下相关 ,一定程度上影响T细胞的激活。

Objective To study the role of DCs in tumor infiltraing T lymphocytes activation in precancerous lesions and carcinoma of the Larynx.Methods 39 specimens, histological diagnosis disclosed precancerous lesions of the larynx in 29 cases and in 10 cases, with squamous cell carcinoma, were performed using antibodies against the cell surface markers S\|100? HLA\|DR?CD45RO, CD45RA by immunohistochemisty.Results S\|100+ cells could be detected in 9 of 10 cases in mild atypical hyperplasia. The infiltration of the atypical hyperplasia was sparse compared with those cases with higher\|graded atypical hyperplasia. The distribution patterns of DCs we observed with severe atypical hyperplasia and squamous cell carcinoma were extremely similar and markedly different from those in grades I and II in specimens. Memory T lymphocytes (CD45RO+) were detected more often in the group with severe atypical hyperplasia(8 of 9 cases) than in the group with squamous cell carcinoma (3 of 10 cases). The inverse became evident for CD45RA immunolabeling. Conclusion Few DCs were found in the mild and moderate precancerous lesions. Severe atypical hyperplasia (grade III)demonstrated an increased infiltration with both dendritic cell and T cells. As the degree of malignancy rising, more DCs infiltrate the tumor, but not DCs may activate T lymphocytes efficient.
