彩色多普勒血流显像在急性阴囊疼痛中的应用

The Clinical Role of the Color Doppler Flow Imaging in the Evaluation of the acute Scrotum

目的 应用彩色多普勒血流显像 (CDFI)评价 68例急性阴囊疼痛患者 ,探讨CDFI的临床应用价值。研究方法 采用直接探测法 ,CDFI取低速血流设置。通过与健侧阴囊CDFI表现比较 ,将患侧阴囊CDFI表现分为正常血供 ,低血供 ,无血供和高血供。结果 以睾丸内无血供诊断睾丸扭转 14例 ,敏感性和特异性均为 10 0 %。以睾丸和附睾内高血供诊断睾丸炎及附睾炎的敏感性为 90 % ,特异性为 10 0 % ,准确性为 91 5 %。结论 :CDFI可准确地区分睾丸扭转和炎症 ,是一种评价急性阴囊疼痛简便、准确、可靠及临床价值较高的方法。

Objective:Color Doppler flow imaging was used to assess 68 patients with the acute onset of serotal pain.Method:Both intra and extratesticular blood flow were read as normal,diminished,absence,or increased compared to the contralateral testicle.Results:Both the sensitivity and specificity of CDFI in the diagnosis of testicular torsion by the complete disappearance of test icular blood flow were 100%.The sensitivity,specificity and accuracy of CDFI in the diagnosis of testicular and epididymal inflammation by the increase in testicular and epididymis blood flows were 90%,100% and 91.5% respectively.Conclusion:Color Doppler flow imaging is a simple,useful and highly accurate diagnostic method in the evaluation of patients with the acute scrotal syndrome.
