摘要
目的 :应用磁共振氢质子波谱(1H -MRS)尝试研究乳腺病变波峰形态学特征 ,以及不同病变代谢物质生化含量的差异 ,以提高乳腺肿瘤的确诊率。材料和方法 :92例病理证实的女性乳腺病变患者 ,均进行了常规MRI检查和MRS测量 ,共计检测了248个病变部位。结果 :乳腺良性病变及正常组织多含一“M”型波峰(Ⅰ型) ,而乳腺肿瘤的 1H -MRS波形中则无(Ⅱ型) ;乳腺恶性肿瘤与良性肿瘤相比 ,胆碱组半波宽度相差显著(p<0.05) ,且前者胆碱含量明显升高(p<0.01) ,同时伴有肌酸含量的升高(p<0.05)。结论 :乳腺肿瘤的 1H -MRS波形与非肿瘤性病变及正常组织的MRS波形明显不同 ,可通过观察将两者区分开来(p<0.01) ;乳腺恶性肿瘤的胆碱含量明显升高 ;1H -MRS与MRI相配合 ,可以提高MRI的确诊率 。
Purpose:To study the morphologic features and metabolic materials content of breast diseases with proton MR spectroscopy(1H-MRS) in order to improve diagnostic accuracy rate of breast tumors.Materials and Method:Routine MRI and single-voxel 1H-MRS were performed in 92 female patients with single or double breasts and total 248 nominal voxel sizes(1cm3) were examined.Results:The 1H-MRS wave patterns are quite different between breast tumors (typeⅡ)and non-tumor tissues(typeⅠ)(p<0.01).1H-MRS has shown a quite higher level of choline in malignant lesions than which in benign tumors (p<0.01).Conclusion:Using typeⅠand typeⅡwave styles,we can distinguish breast tumors from other diseases.It is shown that the choline level in malignant tumor is much more higher than that in benign one.1H-MRS is useful in detecting breast diseases and helpful to improve diagnosis correctly for MRI
