摘要
目的 :通过对原发性高血压肾小球滤过率 (GlomerularFiltrationRate ,GFR)的检测 ,确定核素Gate’s法计算GFR对各期高血压肾功能定量评价的价值。材料和方法 :以99mTc DTPA 14 8MBq静脉注入、SIEMENSORBITER 75 0 0型SPECT对 3 3例正常对照者及 2 8例原发性高血压患者以核素Gate’s法进行了GFR测定。结果 :原发性高血压患者比正常对照组GFR显著降低。Ⅰ期高血压患者与正常对照组GFR比较 ,其减低的程度不如Ⅱ期及Ⅲ期显著。与其他临床指标血肌苷 (Cr)及尿素氮 (BUN)相比 ,GFR与肾功能损害的程度更加吻合。结论 :在确定原发性高血压患者的分期、需对肾功能进行评价时 。
Purpose: To study GFR (Glomerular Filtration Rate) changes in essential hypertension using Gate's method. Materials and Methods: 99m Tc DTPA at dose of 148 MBq was given intravenously and dynamic study was carried out using SIEMENS OBITER 7500 SPECT in 33 normal subjects and 28 patients with essential hypertension. GFR was calculated based on renal uptake. Results: 1. GFR was significantly lower in hypertension group (p<0.01). 2. The decrease of GFR in stage Ⅱ and stage Ⅲ were more severe than in stage Ⅰ. 3. The correlation of GFR and renal function was better than other clinical tests. Conclusion: Detection of GFR by the mothd of nuclear medicine was precise in estimating renal functional damage of essential hypertension.
