AF系统治疗胸腰椎骨折失败原因分析

Analysis of the Reason of Faults in Treatment of Thoracolumbar Vertebral Fracture with AF System

目的 :探讨AF钉治疗胸腰椎骨折的失误及预防。方法 :采用后路减压AF钉固定术治疗胸腰椎骨折。结果 :43例手术中 7例术后出现骨折复位不满意或固定失败。结论 :进钉错误、复位原理错误、过度牵张或张力不平衡和不做横突间植骨融合是失败的主要原因。进钉错误可以通过术中透视加以避免 ,而后 3种错误属于认识上的错误 。

Objective: To discuss the faults of thoracolumbar vertebral fracture treated with AF system and its prevention.Method:To treat of thoracolumbar vertebral fracture with decompression and internal fixation with AF system.Result:7 cases of the 43 case appeared unsatisfactory reduction or unsuccessful fixation.Conclusion:The main reason of failure is misplacement of pedicle screw,fault reduction principle,excessive or imbalance of tension and disapplication of inter transverse process fusion with bone graft.The fault of misplacement screw could be avoided with radiography monitor during operation,the other fault is due to the absent of knowledge,that should be taken enough attention to.
