胰管刷检标本P53蛋白检测对胰管良、恶性狭窄的鉴别诊断

Detecting P53 protein in cytologic specimens from endoscopic pancreatic duct brushing for differentiation of benign and malignant stricture of pancreatic duct

目的 :探讨胰管刷检标本 P5 3蛋白检测对胰管良、恶性狭窄的鉴别诊断价值。 方法 :应用免疫细胞化学的方法检测2 5例 ERCP示胰管狭窄或梗阻病例的胰管刷检标本 P5 3蛋白的表达。结果 :16例胰腺癌性胰管狭窄胰管刷检标本 P5 3蛋白阳性率为 5 6 % ,9例炎性胰管狭窄胰管刷检标本未见 P5 3蛋白阳性表达。胰腺癌胰头部胰管狭窄刷检标本 P5 3染色阳性率(73% )高于体尾部 (2 0 % )。 结论 :胰管刷检标本细胞学检查的同时进行 P5 3检测有助于胰腺癌与慢性胰腺炎的鉴别。

Objective:To investigate diagnostic value of detecting P53 protein in cytologic specimens from endoscopic pancreatic duct brushing for differentiation of benign and malignant stricture of pancreatic duct. Methods: P53 protein in cytologic specimen was detected by immunocytochemistry in 25 patients with pancreatic cancer or pancreatitis.Results: Of the 16 pancreatic cancer patients,9(56%) expressed P53 protein,and none of the 9 chronic pancreatitis patients expressed P53 protein.Positive rate of P53 protein in cancer of pancreatic head was higher than pancreatic body and tail.Conclusion: H E staining combined with detection of P53 protein in cytologic specimens from endoscopic pancreatic duct brushing is useful for differential diagnosis of pancreatic cancer and chronic pancreatitis.
