放射治疗合并肝动脉灌注化疗栓塞治疗原发性肝癌

Clinical Analysis of Radiotherapy Combined with Transcatheter Intra-arterial Infusion Chemoembolization for the Treatment of Primary Liver Cancer

目的 :观察放射治疗 (RT)结合肝动脉灌注化疗栓塞的介入疗法 [简称介入化疗 (IVCT) ]治疗 ,原发性肝癌的疗效。方法 :治疗组 (6 2例 )先行IVCT(化疗方案 :5 Fu、ADM、DDP或 5 Fu、MMC、ADM)一次 ,然后行放射治疗。放射治疗结束后 ,每间隔 1～ 1.5个月再行IVCT一次 ,共 2～ 5次。对照组 (6 8例 )单用IVCT ,化疗方案同治疗组 ,每间隔 1～ 1.5个月一次 ,共 3～ 6次 ,结果 :治疗组总缓解率 (CR +PR)为 77 4 % ,1,2 ,3年生存率分别为79 0 %、6 4 5 %、38 7%。对照组总缓解率为 5 7 4 % ,1,2 ,3年生存率分别为 5 8 8%、39 7%、2 2 3%。两组比较总缓解率和 1,2 ,3年生存率均有统计显著差异 (P <0 .0 5 )。结论 :我们认为RT +IVCT治疗原发性肝癌安全、有效 。

Objective: To study the treatment effect of radiotherapy(RT) combined with transcatheter intra arterial infusion chemoembolization(IVCT) for the treatment of primary liver cancer. Methods:For the treatment group( n =62),a course of RT was given after the 1st IVCT treatment.IVCT treatment was given once every 1～1.5 months,2-5 times in total.In contrast,only the same IVCT treatment method was given for the control group( n =68),totally 3-6 times of IVCT without RT.Results:In the treatment group,the total response rate(CR+PR) was 77.4%,and the 1,2,3-year survival rates were 79.0%,64 5%,and 38.7%,respectively.In the control group,the total response rate was 57.4%,and the 1,2,3-year survival rates were 58.8%,39.7%.and 22.1%,respectively.Our results showed statistical significant difference( P<0.05 ).Conclusion:Our results showed that our method of RT+IVCT is a safe and effective method for the treatment of patients with primary liver cancer.
