腭裂术后患者牙颌结构测量分析

A Cephalometric Analysis of Maxillodental relationships in Adult Patients with Operated Cleft Palate

目的 探讨幼年接受腭裂手术患者牙颌结构的畸形特点 ,分析其发生机制。方法 对 5 5例腭裂术后患者 (手术年龄 4～ 7岁 ,现年龄 18～ 2 5岁 )颌面部形态、牙齿及咬合关系进行临床检查及电子计算机X线头影测量 ,将测量值与正常人资料相比较。结果 腭裂修复术后患者上、下颌切牙突度比正常人显著减小 ,上颌骨长度比正常人显著缩短 ,下颌角变钝。结论 腭裂术后患者牙颌结构存在明显发育畸形 ,其形成机制十分复杂 ,上颌骨发育不足及畸形与手术有关 ,下颌部畸形多为代偿性变化。

Objective To explore the maxillodental defects in adult patients with operated cleft palate and analyse the mechanism in which the defects develop.Methods 32 males and 23 females with cleft palate treated at the ages of 4 to 7 years were studied with Cephalometrics for Orthognathic Surgery(COGS),the datas were compared with that of native Hubei adults with normal occlusions. Results In adult patients with operated cleft palate,there were a decreased angulation of the maxillary central incisor to the NF, a decreased angulation of the mandibular central incisor to the mandible,a shorter length of the maxilla,a more obtuse Go angle,all the differences were significant.Conclusion The patients appeared developed abnormalites.The mechanism was complex.The maxillary hypoplasia was related to the cleft palate and the operation.The mandibular defects resulted from compensative variation.
