动脉导管未闭的手术治疗(附57例报告)

SURGICAL TREATMENT OF PATENT DUCTUS ARTERIOSUS

本文报告了57例动脉导管未闭的治疗。54例单纯动脉导管未闭在双重结扎的基础上贯穿缝扎。其中1例术中导管破裂缝合止血。切断缝合术1例,术中出血死亡。合并房间隔和室间隔缺损各1例,在体外循环深低温低流量下切开主肺动脉缝合导管内口。全组死亡率1.75％。无喉返神经损伤。术后发生高血压脑病1例,随访无再通。

This paper rcported surgical treatmcnt of 57 cases of patent Ductus Arteriosus (PDA). 54 cases of simple PDA were treated by transfixion based on duplex ligation. One of them the PDA ruptured during operation and homeostasis with suture. Division and suture was done in one, and the patient died from bleeding during operation. Trans-pulmonary suture of PDA in a case associated with ASD and a case associated with VSD under deep hypothermia extracorporeal circulation with low flow perfusion. The total mortality was 1.75 percent. Recurrent laryngeal nerve paralysis was not found. There was one case with postoperative hypertensive encephalopathy. Follow-up resuts showed no recurrence.
