低分子肝素治疗不稳定型心绞痛的疗效及其对内皮功能的影响

The effects of low molecular weight heparin on unstable angina and endothelial function

目的 研究低分子肝素 (LMWH)治疗不稳定型心绞痛 (UA)的疗效及其对血管内皮功能的影响。方法 将 44例UA患者随机分为A、B两组 ,B组予常规治疗 ,A组在其基础上加用LMWH 5 0 0 0IU ,1次 /12h ,腹部皮下注射 ,疗程为一周。观察心绞痛缓解、缺血性心电图改善及血浆内皮素 (ET)、血管性假血友病因子 (vWF)的变化。结果 缓解心绞痛的疗效A组优于B组(P <0 .0 5 ) ;治疗前两组ET、vWF含量比较 ,差异无显著性 (P >0 .0 5 ) ,但均高于对照组 (P <0 .0 0 1) ,治疗后两组ET、vWF含量均下降 ,A组降低程度较B组显著 (P <0 .0 5 )。结论 LMWH缓解UA患者的心绞痛症状与血管内皮功能改善有一定关系。

Objective To study on effects of low molecular weight heparin(LMWH)on unstable angina(UA) and endothelial function.Methods 44 cases of UA were randomly dividedd into two groups(A,B).Patients in group A were treated with subcutaneous LMWH 5 000IU twice a day for 7 days in addition to conventional therapy and those in group B were only given conventional therapy. Plasma levels of endothelin(ET)and von Willbrand factor(vWF) were measured in 44 patients with UA before and treatment.Results Remission rate of angina in group A was higher than that in group B( P <0.05). Plasma levels of endothelin(ET) and von Willbrand factor(vWF)in two, groups were higher than those in healthy group. There was no difference in plasma levels of ET and vWF between two groups before treatment( P >0.05, P >0.05,respectively). After treatment plasma ET and vWF levels in two groups were decreased significantly compared with those before treatment.Plasma ET and vWF levels in group A were significantly lower than those in B group after treatment ( P <0.05, P <0.05,respectively).Conclusion The relief of angina in group A is related to the improvement of endothelial function.
