干燥综合征合并节段性不出汗1例

Sjgren's syndrome associated with partial absent sweating:a case report

患者女性,56岁,眼口干燥半年伴左面部、胸部、上臂不出汗。查体:瞳孔直径右1mm,正三角形;左3mm,不规则,略呈三角形。左胸面部针刺感减弱,皮温降低,左手大小鱼际肌萎缩。实验室检查:类风湿因子阳性,左眼泪液试验阳性,左面部、胸背部发汗试验示无汗,下唇粘膜活检示部分腺体萎缩,细胞浸润。符合功能型无汗。颌下腺造影示导管排空功能差。诊断为干燥综合征合并偏侧不出汗,符合Ross综合征顿挫型。

A 56 year old female presented with dehydration of eyes and mouth for half an year, and absent sweating of left face, breast and arm. Physical examination:diameter of right pupi was 1mm, right triangle; left, 3mm, irregular. prickly sensation was weak with low temperature on the surface of face and left thorax. Left hypothenar and hyperthenar were myatrophy. Laboratory examination:RF was positive. Little left tears and absent sweating of left face and breast were seen. Pathology(mucosa of lower lip):gland atropy, cell infiltration. Submandibular contrast examination showed difficult duct empty. Diagnosis:Sjgrens syndrome with partial absent sweating, abortive type of Ross' syndrome.
