基质金属蛋白酶-2在肺肿瘤中的表达及其预后意义

EXPRESSION OF MMP-2 IN LUNG NEOPLASMS AND ITS PROGNOSTIC VALUE

目的 :研究原发性肺癌中基质金属蛋白酶 - 2 (MMP - 2 )的表达与肺癌不同病理类型、TNM分期的关系及其预后的意义。方法 :用已知阳性的乳腺癌组织切片作为阳性对照 ,选择免疫组织化学染色方法对 65例肺癌组织中MMP - 2的表达水平进行检测。结果 :MMP - 2在所有肺癌组织中均有表达。腺癌组和腺鳞癌组中MMP - 2的表达水平显著高于鳞癌组 (P <0 .0 1 ) ,但在腺癌组与腺鳞癌组中的表达差异不明显 (P =0 .2 33)。MMP - 2的表达水平随TNM分期增加而增加 ,其中Ⅰ期与Ⅲa期、Ⅰ期与Ⅲb期、Ⅱ期与Ⅲb期之间相比有显著性差异 (P <0 .0 1 ) ;Cox ,s-比例风险模型多因素分析提示患者的生存期随着MMP - 2表达的增高而降低。结论 :肺癌组织中MMP - 2的表达与肺癌的TNM分期和预后有关 ,提示MMP - 2的表达水平可能是评估肺癌预后的指标。

Objective:To investigate the correlation of the expression of MMP-2 with its pathological type,TNM stage and its prognostic value of primary lung cancer.Methods:The expression of MMP-2 was detected in 65 cases of lung cancer tissues by immunohistochemical method,and breast cancer tissues used as positive control.Results:All of the 65 cases expressed MMP-2.The expression level of MMP-2 in the adenocarcinoma group and adenosquamous carcinoma group were remarkably higher than that of squamous cell carcinoma group ( P <0.01, P <0.01),but without statistics difference ( P =0.233) in the adenocarcinoma group and adenosquamous carcinoma group.With the increases of TNM stage,the expression of MMP-2 increased,and there were statistics difference between Ⅰ and Ⅲa stage,Ⅰ and Ⅲb stage ( P <0.01, P <0.01),and so were Ⅱ and Ⅲb stage ( P <0.01).Cox's multivariate proportional hazards analyes reveled that high carcinomatous MMP-2 values were of prognostic significance for a poor survival of the patients.Conclusions:The expression of MMP-2 correlated with histological grading and metastasis might be regarded as a parameter for evaluating the prognosis of primary lung cancer
