摘要
目的 :探讨反流性食管炎 (Reflaxesophagitis,RE)的食管动力学改变特征及反流性食管炎的发生机理。方法 :使用高分辨的多道灌注测压系统 (PCpolrrafHR瑞典产 )测定了 12例正常人及反流性食管炎患者 2 8例的食管压力。结果 :RE组食管下括约肌 (LES)静息压 (2 41± 0 87kPa)明显低于正常对照组 (3 0 3± 0 6 2kPa) ,食管体蠕动波幅 (2 81± 0 43kPa)明显低于正常对照组 (3 2 3± 0 6 2kPa) ,食管体部运动异常检出率 (71 43% )明显高于对照组 (16 6 7% ) ,差异均有显著性 (p <0 0 5 ) ;下食管括约肌长度 (3 11± 0 47cm)与正常组 (3 35± 0 34cm)比较 ,差异均无显著性 (p >0 0 5 )。结论 :LES压力低下 ,食管运动功能紊乱、LES的松驰可能在RE的发病中起重要的作用。
Objective:To investigate the characteristic changes of esophageal motility in order to realize mechanism of reflux in reflux esophagitis(RE).Methods:The polygraf HR manometry exmination system and esophageal manometry were studied in 12 healthy persons and 28 patients with refux esophagitis.Results:the rest pressure of LES(2 41kPa±0 87kPa)and peristaltic amplitude of the esdophagus(2 81kPa±0 43kPa)in the patients were significantly lower than that in the control group(3 03kPa±0 62kPa,3 23±0 62kPa,P<0 05),the acidence of the moter disorder of esophageal body(71 43%)in the patients was significantly higher than that in the control group(16 67%,P<0 05);the length of LES(3 11cm±0 47cm)was not significant difference from that of the control group(3 35cm±0 34cm,P<0 05).Conclusion:Lowed LES resting pressure,lower esophageal sphinter relaxation,the moter disorder of esophageal body may pay an important role in the pathogenesis of reflux esophagitis.The result suggest that the esophageal manometry is useful in diagnosis of disorders of esophageal motility and in the study of pathophysiologic mechanism of RE.
