脑梗死及多梗死性痴呆患者血清神经元特异性烯醇化酶的测定 被引量:4

摘要 目的研究脑梗死(CI)及多梗死性痴呆(MID)患者血清中神经元特异性烯醇化酶(NSE)的变化及意思。方法对72例CI患者及55例MID患者血清中NSE浓度进行测定。智能水平采用简易智能量表(MMSE)测定,并参考DSM-Ⅳ诊断标准及Hachinski缺血评分。结果MID急性发作组及CI急性期组患者血清NSE浓度明显高于对照组(P<0.01);MID稳定期组患者血清NSE浓度低于对照组(P<0.05);CI恢复期组患者血清NSE的浓度与对照组无显著性差异。血清NSE的浓度与MMSE评分无显著相关(P>0.05)。结论血清NSE的含量变化可动态反应MID患者的脑组织功能状态。但是NSE并不是一种智能因子,并不能直接反应MID患者的智能状态。 Objective To study the changes of tumor necrosis factor alpha(TNF α)and neuron specific enolase(NSE) in cerebrospinal fluid (CSF) and its clinical significance in patients with multi infarct dementia(MID) .Methods The concentration of CSF TNF α was determined by RIA,and the concentration of CSF NSE was determined by ELISA in 55 patients with MID;Compared with Seventy two patients with cerebral infarction(CI) and 32 patients with functional disease of nervous system(control group).Results CSF TNF α or NSE concentration in patients with MID acute episode group was significantly higher than that in control group respectively(P< 0.01); CSF TNF α concentrations in MID stationary phase group was higher than that in control group(P< 0.05);No significant difference was found in CSF TNF α concentration between MID stationary phase group and CI recovery phase group(P >0.05).CSF NSE concentration in MID stationary phase group was lower than that in control group(P< 0.05);There was no significant correlation between MMSE scores and CSF TNF α or between MMSE scores and CSF NSE concentration in each groups(P >0.05).Conclusion CSF TNF α and NSE concentration of all patients with MID did not present concordant changes at all.The changes of CSF TNF α and NSE concentrations could respond the brain neuroendocrine and immunological changes objectively in patients with MID.However,the changes of TNF α,NSE could not directly indicate the intellectual status of MID patients; for it was not a kind of intellectual factor.
