细胞外基质在肝癌侵袭和转移中的变化

CHANGES OF EXTRACELLULAR MATRIX IN HEPATOCELLULAR CARCINOMA INVASION AND INTRAHEPATIC METASTASIS

摘要 探讨肝细胞癌 (hepatocellular carcinoma,HCC)细胞外基质 (extracellular m atrix,ECM)层粘蛋白 (laminin,L N)、胶原蛋白 (collagen ,Col )的分布表达及其临床意义。利用免疫组化结合计算机图像分析技术观察 5 4例 HCC组织中 L N、 Col 的分布特点和表达水平。结果表明 L N和 Col 的分布方式相似。低分化、侵袭性和伴肝内转移的 HCC其 ECM分布呈中断的索周型和血管型 ,而高分化、非侵袭性及无肝内转移的 HCC的 ECM分布主要是连续的索周型 (P<0 .0 1)。低分化 HCC的 L N、Col 表达水平均低于高分化 HCC(P<0 .0 5和 P<0 .0 1)。侵袭性和伴肝内转移的 HCC其 L N、Col 表达水平分别低于非侵袭性和不伴肝内转移的 HCC (P<0 .0 1)。提示 HCC组织 ECM的分布和表达与肿瘤的侵袭转移密切相关 。 To investigate the changes of distribution and expression of laminin (LN) and collagen Ⅳ (Col Ⅳ) in hepatocellular carcinoma (HCC) and their clinical significance, the distribution and expression of LN and IVC were detected by immunohistochemical method in combination with computerimaging analytic technique in 54 patients with HCC. The results showed that LN was similar to IVC in distribution ways. The interrupted peritrabecular type and vascular stroma type were mainly found in HCCs with poorly differentiation, invasion and intrahepatic metastasis. However, the continous peritrabecular type was chiefly discovered in HCCs without invasion, intrahepatic metastasis and well differentiation ( P <0 01). Moreover, the expression level of LN and IVC in poorly differentiated HCCs was significantly lower than that in well differentiated HCCs ( P <0 05 and P <0 01). HCCs without invasion and intrahepatic metastasis had a significantly higher expression level of LN and IVC than those with invasion and intrahepaic metastasis, respectively ( P <0 01). The results indicated that the distribution and expression of ECM were closely related to HCC invasion and metastasis, and ECM as a structural barrier may play an important role in the repression cancer invasion and metastasis.
