寰枢关节失稳状态下体位对移位方向的影响及临床应用

摘要 目的 :探索和观察寰枢关节失稳状态下体位对移位方向的影响 ,为提高寰枢融合术的成功率提供理论依据。 方法 :通过静力分析确定寰枢关节相对稳定的体位。 结果 :30 0例寰枢后路钢丝固定植骨融合术 ,2 70例于中立位融合获得 95 %的成功率。而未能于中立位融合的 30例失败率达 5 0 %。 结论 :寰枢关节失稳状态下体位对移位方向产生极大的影响 ,中立位是相对稳定的体位。 Objectives:To assess the effects of cervical position on the direction of displacement in unstable atlantoaxial joint and to offer the theoretical basis for posterior arthrodesis to facilitate fusion. Methods:Relative stable position was determined by static mechanical analysis. Results:Of 300 posterior atlantoaxial arthodesis with wiring and bone graft, 270 cases were fixed in neutral position and achieved a 95% success rate. And those failed in fixing in that position had a 50% failure rate. Conclusions:Cervical position may has significant effects on the direction of displacement in atlantoaxial joint under unstable state. The neutral position is the relative stable one. Fixation and fusion in this position may get a higher success rate.
