多普勒超声心动图对高血压性心脏病左室舒张功能的诊断价值

Evaluating the Diagnosis Significance of Doppler Echocardiogram on Hypertensive Heart Disease LV Diastolic Function

目的 探讨多普勒超声心动图对高血压性心脏病左室舒张功能的诊断价值。方法 用多普勒超声心动图测定 80例原发性高血压 (EH)患者和 80例正常人心脏功能多项参数 ,并做相关性分析。结果 ①EH组PVA增大 ,PVE PVA比值缩小 ,IRT延长 ,LVmass和LVMI增加 ,LAD增大 ,均与对照组相关显著 (P <0 .0 5 )。②MAP与PVE PVA ,IRT ,LVmass均有相关性。③LVmass与PVE ,PVE PVA ,IRT ,LAD相关性好。④IRT与PVE PVA正相关。结论 高血压性心脏病患者早期即可发生左室舒张功能降低 ,伴有LAD增大、LVmass增加、LVMI增高和室壁肥厚 ,以上参数均可作为高血压性心脏病的检测指标。

Objective To evaluate the diagnosis significance of Doppler echocardiogram on hypertensive heart disease left ventricular diastolic function.Method 80 cases of essential hypertension (EH) and 80 cases of health volunteers were interrelatively studied with Doppler echocardiogram.Result ①PVA increased,the ratio of PVE and PVA decreased,IRT lengthen,LVmass and LVMI increased and LAD enlarged in EH group,there was singnificant difference compared with the control group (P<0.05);②PVE/PVA,IRT and LVMI were closely related to MAP;③PVE,PVE/PVA,IRT and LAD were closely related to LVmass;④PVE/PVA was negatively reated to IRT;⑤LAD was positively related to PVA.Conclusion The abnormal of diastolic function of LV was found in early stage EH,followed with the enlage of LAD,increase of LVmass,increase of LVMI and wall.All the criterion mentioned above were criterion in EH cardiac disease.
