TURP前后前列腺超声形态学变化及意义探讨

The study of ultrasound morphology in prostate before and after TURP

目的 观察前列腺增生患者在经尿道前列腺电切术(TURP)前后前列腺各径线和截面积的改变 ,研究手术前后前列腺的形态学变化 ,用以指导TURP的手术操作。方法 手术前后用经直肠纵向超声断层法测定前列腺各径线长度和截面积并进行比较 ,同时观察和记录前列腺形态的变化。结果 术后前列腺截面积缩小、前后径缩短、上下径延长(均P<0.01)。前列腺截面积缩小与前后径缩短呈显著正相关(P<0.01) ,上下径延长幅度与前后径缩短幅度之比为(1.0±0.3)∶(0.7±0.2)。结论 经直肠纵向超声断层法能直接观察前列腺在TURP前后形态学的变化 ,对开展TURP有一定的帮助。

Objective To investigate the morphological change of prostate through before and after transurethral resction of prostate(TURP) in the patients with hyperplasia of prostate using ultrasounic echography.Methods To detect and compare the meridians and section cut straight through before and after operation with vertical ultrasound setion through rectum,and to observe and record the change of prostate.Results The section cut straight through was dwindled,the occipitofrontal diameter was shorted and the superior and inferior diameter was elongate(P<0.01).There was a positive correlation(P<0.01) between the dwindled section cut straight through and the shorted occipitofrontal diameter.The ratio between the elongated superior and inferior diameter and the shorted occipitofrontal diameter was (1.0±0.3)∶(0.7±0.2).The majority were fine of miction after operation,some present remnant valve,pseudo_pathway and urinary incontinece.Conclusion The prostate morphological change can be observed directly before,in and after operation by this method,the method may be useful for guiding TURP and evaluating results after operation.
